If so, you are not alone.

There are many different causes to scleral lens discomfort. Most of the time, it is due to the fit of the scleral lens.

If the scleral lens does not fit your eye perfectly, there can be small issues with the lens that can create comfort problems. Most of the time, the issue is with the edge of the scleral lens. Scleral lenses have three different zones that are evaluated and adjusted.

The center of the lens is called the central zone, then outside of that is the midperipheral or limbal zone, and lastly is the edge. The edge is usually the culprit of discomfort issues.

In this particular case, the patient complained that they felt discomfort at the upper and outer portion of the right eye when they were wearing their scleral lens. If you look carefully at the image of the eye, you can see a VERY small cyst in the photo.

That tiny little cyst was interacting with the edge of the lens and rubbing it.

You can view this in the OCT image with the yellow arrow.

To solve this issue, we are going to decrease the overall diameter of the scleral lens, in hopes to eliminate the edge of the scleral lens from interacting with the cyst.

Key takeaway: If a patient is able to pinpoint their discomfort, look very carefully with different instruments to identify the cause.
